Back pain would have just been too easy.

Answer: ankylosing spondylitis (Marie Stumpell disease, Bechterew disease)

This abdomen radiograph shows many good examples of classic findings of ankylosing spondylitis. 1) diffuse syndesmophyte ankylosis (ie “bamboo spine”), 2) interspinous ligament ossification (so called “dagger spine”), 3)  of course SI joint ankylosis and 4) pubic symphysis ankylosis

Any other spondyloarthropathies in the differential?
