Case 34 Answer: tracheal diverticulum a.k.a. paratracheal air cyst.

No need to hedge on this one. It is incidental - classic location and appearance. Hedging is only going to turn this into a big issue when it is NORMAL.

Coronal MiniP from the same patient (having fun with Intellispace - if you don’t know what a MiniP is, it’s the reverse of a MIP, thus using the LOWEST density instead of the highest)

These are NORMAL VARIANTS, generally located at the level of T2 and almost always at the 7-o'clock position of the trachea
