Answer: Monteggia fracture-dislocation ( mid ulnar fracture with radial head dislocation)

side note (and a semantics peeve): remember a fracture-dislocation is a defined as: severe injury where a fracture and dislocation both occur simultaneously. Reports often come over saying “Impression: No fracture-dislocation”, when you probably mean to say “No fracture or dislocation”. :)
